এখানে খেয়াল করুন যে আইডি কার্ড একটি আইডি কার্ড এবং পাসপোর্টের স্যাম্পল আমি আপনাদেরকে দেখাইতেছি। অনেক সময় দেখা যায় নামের মিল থাকে না। পিতার নামে মিল থাকে না অথবা মাতার নামে মিল থাকে না অথবা জন্ম তারিখে মিল থাকে না। গ্রাহক এসে যদি আমাকে বলে যে ভাই আমি একটি পাসপোর্টে ফরম ফিলাপ করব। পাসপোর্ট ফরম ফিলাপ করে দেন। ব্যাংক জমা দিয়ে দেন। তখন আমি জিজ্ঞেস করে কত টাকা? তখন আমি বলি যে ভাই ফরম ফিলাপ তো মূল বিষয় না। আপনি আইডি কার্ড আর পাসপোর্টটি আমাকে দেখান। কারণ এটি মিল আছে কিনা সেটি আমাকে দেখতে হবে। যে শুধু ফর্ম ফিলাপ করলেই হবে না এখানে সাথে কোন অঙ্গীকারনামা লাগবে সেই বিষয়টি আগে কনফার্ম হইতে হবে। তখন পাসপোর্ট আর আইডি কার্ড আমি হাতে নিয়ে প্রথমে নাম মিলাই, পিতার নাম মিলাই, মাতার নাম মিলাই, জন্ম তারিখ মিলাই এবং জন্মস্থান মিলাই এই বিষয়গুলি আগে কনফার্ম হই। যখন মিল থাকে না তখনই একটি অঙ্গীকারনামা দিতে হয়। পাসপোর্টের তথ্য পরিবর্তন বিষয়ক একটি অঙ্গীকারনামা গ্রাহকের পক্ষ থেকে পাসপোর্ট অধিদপ্তর বরাবর একটি অঙ্গীকারনামা লিখতে হয়। তখনই এই পাসপোর্টের ফর্ম ফিলাপের পর ব্যাংক জমা দেয়ার পর এই অঙ্গীকারনামাটিও সংযুক্তি দিতে হয়।

এই অঙ্গীকারনামাটি কিভাবে লিখবেন সেটি আমি এখন আপনাদেরকে দেখাবো। মূল বিষয়তে চলে যাই। মাইক্রসফট ওয়ার্ড এ অঙ্গীকারনামাটি লিখবেন। যে যেভাবে লিখতে পারেন লিখবেন অসুবিধা নেই। সর্বপ্রথম হেডিং এ থাকবে অঙ্গীকারনামা। এরপর থাকবে পাসপোর্টের তথ্য পরিবর্তন অবলিক সংশোধন সংক্রান্ত। এই হলো হেডিং। এরপরে মূল বক্তব্যের প্রথম প্যারা। এখানে অবশ্যই আইডি কার্ডের সাথে মিল করে নাম, পিতার নাম এবং মাতার নাম দিয়েই এই অঙ্গীকারনামাটি শুরু করতে হবে। আমি একটি স্যাম্পল দিয়েছি আমি রফিকুল ইসলাম পিতা ইদ্রি শেখ মাতার রোকেয়া বেগম ঠিকানা এই ডাকঘর থানা উপজেলা জেলা পেশা চাকরি যে যেই পেশা ডাক্তার হলে ডাক্তার চাকরিজীবী হলে চাকরিজীবী ছাত্রছাত্রী হলে ছাত্রছাত্রী ব্যবসায়ী হলে ব্যবসায়ী যে যে পেশা সেই পেশাটি এখানে লিখবেন ধর্ম ইসলাম যার যার ধর্ম তিনি লিখবেন জাতীয়তা বাংলাদেশী জাতীয় পরিচয়পত্র নম্বর অবলিক জন্মনিবন্ধন নম্বর ব্র্যাকেটে অপ্রাপ্ত বয়স্কদের ক্ষেত্রে এই পদ্ধতিতে এজন্যই লেখা হয়েছে এটি একটি ফরমেট সকলের জন্য সকল পাসপোর্ট আবেদনকারীদের জন্য যেন এই ফরমেটটি কাজে লাগে। অর্থাৎ যার জাতীয় পরিচয়পত্র রয়েছে সে জাতীয় পরিচয়পত্র নম্বর এখানে লিখবেন। যার জন্মনিবন্ধন রয়েছে তিনি জন্মনিবন্ধন নম্বর এখানে লিখবেন। জন্মনিবন্ধন নম্বর থাকলেই যে জন্মনিবন্ধন নাম্বার লিখতে হবে তা না। পাসপোর্টের পাসপোর্টের ফরমে আপনি যেটি এন্ট্রি করেছেন সেটি এখানে লিখবেন।

এরপর এই মর্মে অঙ্গীকার করিতেছি যে আমার পূর্ববর্তী পাসপোর্ট নম্বর এত। পরবর্তী প্যারায় লিখতে হবে আমার পূর্ববর্তী পাসপোর্ট নম্বর এত। অর্থাৎ সর্বশেষ আপনার বইটি বইয়ের নম্বরটি এখানে লিখতে হবে। অনেকে কিন্তু বই লুকান। আরো একটি বই থাকে বিদেশে বই ফালায় আসেন সেই বইটির কথা উল্লেখই করে না। মনে করেন যে ওটি উল্লেখ না করলে কোন অসুবিধা নেই। কিন্তু সেক্ষেত্রে কিন্তু আপনি অসুবিধায় পড়বেন। সর্বশেষ বই নম্বরটি এখানে লিখতে হবে এবং বইয়ের ইস্যু তারিখ। সেই বইটি কত তারিখে তৈরি করেছিলেন সেই তারিখটিও এখানে লিখতে হবে। ইস্যু তারিখ এততে আবেদনের সময় আমার নিজ নাম অবলিক পিতার নাম অবলিক মাতার নাম অবলিক জন্ম তারিখ ভুল লিপিবদ্ধ করা হয়েছে অর্থাৎ যেহেতু এটি একটি অঙ্গীকার নামার ফরমেট তাই এখানে সকল বিষয়ে চলে আসবে যার যেটি ভুল তিনি সেটি কিন্তু নিম্নুক্ত ছকে লিপিবদ্ধ করবেন ফরমেটটি সকলের জন্য প্রকৃতপক্ষে আমার জাতীয় পরিচয়পত্র অবলিক জন্মসনদ ব্র্যাকেটে অপ্রাপ্ত বয়স্কদের ক্ষেত্রে অনুযায়ী আমার নিজ নাম অবলিক পিতার নাম অবলিক মাতার নাম নাম অবলিক জন্ম তারিখ এবং পাসপোর্টে উল্লেখিত নিজ নাম অবলিক পিতা নাম অবলিক মাতা নাম অবলিক জন্ম তারিখ নিম্নরূপ এই হলো ফরমেট এই ফরমেটটি লেখার পর আপনাকে চারটি কলাম বিশিষ্ট একটি টেবিল তৈরি করবেন এবং উপরে হেডিং এ বাম পাশে দুইটি কলামকে একটি গ্রুপ হেডিং নিবেন বর্তমান পাসপোর্টে প্রদর্শিত ভুল তথ্য লিখবেন এবং ডান পাশে দুইটি কলামের একটি গ্রুপ হেডিং নিবেন সেখানে লিখবেন জাতীয় পরিচয়পত্র অবলিক জন্ম নিবন্ধন অনুযায়ী প্রকৃত তথ্য অর্থাৎ বাম পাশে পাসপোর্টের তথ্য বাম পাশে থাকবে আর ডান পাশে জাতীয় পরিচয়পত্র এনআইডি কার্ডের তথ্য ডান পাশে থাকবে এনআইডি কার্ডে যেই তথ্যগুলো রয়েছে সেগুলো ডান পাশে লিখতে হবে নাম পিতা নাম মাতা নাম জন্ম তারিখ সবগুলি লিখতে হবে তা না আপনার যেটি সংশোধন করবেন আপনি শুধু সেটি লিখবেন যদি এমন হয় যে আপনার ক্ষেত্রে শুধু নামের সমস্যা ঠিক এভাবে আপনি শুধু নামটি লিখবেন যার যেটিতে সমস্যা তিনি সেটি লিখবেন এই ব্যক্তির আমি যেটি ফরমেটটি লিখেছি তার চারটি বিষয়ে সমস্যা ছিল নাম, পিতার নাম, মাতার নাম এবং জন্ম তারিখ। তাই চারটি বিষয় উল্লেখ হয়েছে। ঠিক একইভাবে আপনি আপনার বিষয়গুলো উল্লেখ করবেন। করে নিচে লিখবেন উপরযুক্ত অঙ্গীকারনামার যাবতীয় তথ্য সঠিক। ইহার কোন অংশ মিথ্যা নয়। আমি কোন তথ্য গোপন করি নাই। কোন মিথ্যা তথ্য দেয়া থাকিলে এই পরিবর্তন বা সংশোধনজনিত কারণে ভবিষ্যতে কোন প্রকার আইনগত জটিলতা হইলে আমি দায়ী থাকবো। আমি স্বেচ্ছায় সজ্ঞানে উপযুক্ত অঙ্গীকার নামার যাবতীয় মর্ম সম্মক অবগত হইয়া স্বাক্ষর প্রদান করিলাম। এরপর স্বাক্ষর দিতে হবে।

আবেদনকারী অপ্রাপ্ত বয়স্ক হলে আবেদনকারী যদি 18 বছর নিচে হয় সেক্ষেত্রে অভিভাবকের সাক্ষর প্রয়োজন পড়লে অভিভাবক সাক্ষর দিবে অসুবিধা নেই। এরপর তারিখ লিখবে মোবাইল নাম্বার দিয়ে।

এই অঙ্গীকারনামাটি আপনাকে পাসপোর্টের ফরমের সাথে সংযুক্ত করে দিতে হবে। আমাদের অঙ্গীকারনামা লেখা হয়ে গেল। আশা করি অঙ্গীকারনামা কিভাবে লিখতে হয় আপনারা বুঝতে পেরেছেন।

এখন করণীয় কি কি লাগবে আপনার পাসপোর্টটি তৈরি করতে গেলে কি কি লাগবে পাসপোর্টের ফরম তিন পাতা বিশিষ্ট যে ফরম সেই ফরমটি আপনার লাগবে সামারি শিট লাগবে ব্যাংক জমা রশিদ লাগবে এনআইডি কার্ডের ভেরিফাই কপি লাগবে ভেরিফাই কপি এইজন্যই লাগবে আপনি যে পরিবর্তন চাইতেছেন সেটি সার্ভারে আছে কিনা ইনস্ট্যান্ট সেগুলা কিন্তু অফিসাররা আপনার আবেদনটি অনুমোদন দেওয়ার সময় কিন্তু ভেরিফাই কপি তাদের দেখতে হয় এজন্য এনইডি কার্ডের ভেরিফাই কপি নিয়ে যাবেন অঙ্গীকারনামা নিতে হবে এনইডি কার্ডের ফট ফটোকপি নিতে হবে। পাসপোর্টের ফটোকপি নিতে হবে। সাথে অবশ্যই মূল পাসপোর্ট ও এনআইডি কার্ড থাকতে হবে। পাসপোর্টের ফর্ম ফিলাপ কিভাবে করতে হয় আমার চ্যানেলে ভিডিও পাবেন। সুন্দরভাবে সঠিক নিয়মে আমি পাসপোর্টের ফর্ম ফিলাপ করা দেখিয়েছি। সেটি দেখে নিবেন। পাসপোর্ট বিষয়ক নানাবিদ ভিডিওই কিন্তু আমার চ্যানেলে পাবেন।
